    • $450 Team Fee (for each team)

      • Checks from the participating church please
      • Checks made out to: Independent Church League or ICL
    • 2021 Team Registration Form
      • Important: Include any dates team will be unable to play
      • Include primary and secondary coaches with contact information
      • Registration must be signed by a church leader
    • Required prior to first game of play
      • ICL – Team Roster – changes and additions can be made up to half way through the season
      • Waivers (2) for all players under 18 years of age
        • USSSA Washington State: Parental/Guardian Waiver Form
        • Independent Church League – Amateur Minor Athletic Waiver – Release of Liability
      • Background Check Form – All coaches who have minor players on the roster
        • No fee required
        • Can be mailed to Rhonda Tevis (information on form)
        • Note: good for two years
